What to Expect with Medical Abortion

Medical abortion uses two drugs, mifepristone and misoprostol, to end a pregnancy, typically at home. It’s FDA-approved for up to ten weeks of pregnancy. You start with mifepristone, which blocks progesterone to stop the pregnancy from growing. Then, misoprostol is taken, triggering cramps and contractions to expel the pregnancy through your vagina. 

Expect vaginal bleeding (sometimes heavy) and abdominal cramping, along with possible side effects like nausea, chills, or diarrhea. Risks include infection, heavy bleeding, or an incomplete abortion that might need a follow-up procedure. It’s less invasive than surgical abortion, but it’s only available early in pregnancy.

What to Expect With Surgical Abortion

Surgical abortion happens in a clinic or hospital, often later in pregnancy. It involves dilating your cervix so a doctor can use suction, scraping tools, or forceps (for later stages) to remove the pregnancy from your uterus. 

Early on, it’s mostly suction; later, it’s more complex. You might get sedation or anesthesia. The process usually takes one visit. You may have to wait in a recovery room for a few hours to check for excessive bleeding or other issues. 

Risks include cervical damage, uterine perforation, infection, or scarring inside the uterus. It’s a more invasive approach that comes with considerations.

Why Timing Matters

How far along you are (your gestational age) determines which method might be an option. 

Medical abortion is available early in pregnancy; surgical abortion is available through most of pregnancy, depending on state laws. Both have side effects and risks, and neither is a one-size-fits-all procedure.
